Introduction
Matthew Baggott and Professor Lynn Robertson at the Helen Wills Neuroscience Institute of the University of California at Berkeley are working with the moderators of Erowid to carry out a survey of people who have used psychedelics. You should only participate in this survey if you have used psychedelics (also called hallucinogens or entheogens) in the past. This includes LSD (acid), psilocybin (magic mushrooms), MDMA (ecstasy, E, X), Ketamine ("K" or "vitamin K"), Cannabis (marijuana, pot, hashish) and others. It should take between 5 and 25 minutes to fill out this survey.

The main purpose of this survey is to find out about how psychedelics can affect vision.
The researchers will present the survey results to other scientists at conferences and in scientific journals. They will also summarize the results on the web where you and others can read them.

This survey is anonymous.
This means that the researchers will not collect any information that could be used to identify you. This questionnaire and any answers you provide will be sent from your computer to the website in an encrypted ('scrambled') form, making it difficult for anyone to intercept while it is being sent. This encryption also makes it difficult for anyone monitoring the Web to distinguish people who are filling out this survey from others who are just browsing. However, you should be aware that programs exist that can record what is on your screen before it is encrypted and sent. These programs are most often used by computer owners (such as businesses) to make sure their machines are not being used in a way that they do not wish.

Consider whether it is appropriate to answer this questionnaire from the computer you are using.
You should also consider whether you are comfortable with the amount of privacy you have and whether it is possible that you might be interrupted by someone you would prefer not to see your answers.

No computer security or privacy system is foolproof.
There is the small possibility that someone could use one of these methods to see your answers to this survey. If you have strong concerns about being identified as someone who has used psychedelics, you should not fill out this survey.

Contact Information.
At the end of the questionnaire, you will be given the option of sending an email to the research team so that they can contact you about further research. Sending email is optional; you can participate without doing this. If you give your email address, it will be kept confidential by the researchers, and your email address will not be connected with the answers you have given. Your participation in this research is voluntary. You are free to refuse to take part. You may refuse to answer any questions and may stop taking part in the study at any time.
