I've had many experiences with opioids, and wasn't thrilled about tapentadol when the literature was made public, but since I had acquired a script, I've taken some mental notes. This medicine is still new-ish and not heavily prescribed, as well as it shouldn't be. Tapentadol seems like an alternative opioid for people who cannot tolerate the -codone family.

Medication used: Nucynta ER 100 (Tapentadol extended release 100 mg).

My tolerance is equal to ~20 mg oxycodone, ~25 mg hydrocodone, ~175 mg tramadol. Tramadol does not provide any euphoria for me and is just used for analgesia.

100 mg ER formulation provided me 15 - 20 minute 'bursts' of pain relief with no euphoria or CNS effects. The pain relief lasted ~5 hours for me - I have found I can safely redose after 8 hours instead of 12.

200 mg ER formulation provided average pain relief, again in on again, off again rounds. No euphoria to speak off, but a little anxiety, tingling in extremities/fogginess (possibly an effect of tapentadol's secondary mode of action). I would compare 200 mg of the ER formulation to 12 - 16 mg hydrocodone. Nucynta ER's pharmaceutical parent gives tapentadol a 5:1 tapentadol to oxycodone equianalgesic portfolio, but I disagree. I would equate 100 mg IR tapentadol with 8 - 12 mg hydrocodone or 6 - 10 mg oxycodone. As far as activity at the mu opioid receptor, it feels very dull and dirty, similar to a moderate dose of fentanyl or a high dose of tramadol.

Strangely enough, I felt that tapentadol has significant affinity for k-opioid receptors, but maybe I'm mistaking NRI activity for that.

After two weeks, I discontinued tapentadol and avoided withdrawal by taking tramadol 24 hours after my last dose of tapentadol, careful to space out doses of tapentadol and tramadol by no less than 18 hours. I would avoid any combinations with this shit, it's pretty dirty.

I cannot recommend tapentadol for those seeking an opioid high. It simply falls flat. However, its dual method of action makes it novel for pain relief and depression. I am worried that the withdrawals from tapentadol will be extremely difficult if not tapered down.
